These terms govern your use of evertonwellness.com, operated by Everton Health and Wellness Physician Assistant Inc. (“Everton,” “we,” “us”). By using this site you agree to them. If you do not agree, please do not use the site.
Agreeing to these terms
By accessing or using this website you accept these terms and our Privacy Policy. We may update them; the date above shows when they last changed, and continuing to use the site after a change means you accept the updated terms.
Who may use this site
This site is intended for adults 18 or older. Everton’s clinicians are licensed in California and care is offered only to people located in California. You may read the site from anywhere, but submitting an invitation request indicates you are seeking care in California.
This site is not medical advice
Everything on this site is general information about our practice. It is not medical advice, diagnosis, or treatment, and it is not a substitute for care from a qualified clinician who knows your history. Never disregard or delay seeking professional medical advice because of something you read here.
No provider–patient relationship
Using this site, submitting the invitation form, or emailing us does not create a provider–patient relationship. That relationship begins only if we accept you as a patient, you complete our intake and consent process, and care formally starts.
Pre-launch status
Everton is not yet accepting patients. The invitation form collects expressions of interest so we can contact you when we open. Submitting it does not reserve a place, guarantee acceptance, or commit either of us to anything.
Invitation requests
We review every request personally and aim to respond within two business days, but we do not guarantee a response, acceptance, or any particular timeline, and we may decline any request. Please do not send detailed medical information through the form — it is not a secure channel for clinical detail.
Acceptable use
Please use the site lawfully. Do not attempt to gain unauthorized access to it, disrupt it, scrape it in bulk, submit false information or another person’s personal information, or use it to send spam or malicious content.
Our content
The text, design, wordmark, and images on this site belong to Everton or our licensors and are protected by copyright and trademark law. You are welcome to read the site and link to it; you may not copy, republish, or reuse its content commercially without our written permission.
Links to other sites
Where we link to outside resources — published clinical guidelines, for example — we do not control them and are not responsible for their content or their privacy practices.
No warranties
The site is provided “as is” and “as available.” We work to keep it accurate and running, but we do not warrant that it will be uninterrupted or error-free, or that the information on it is complete or current.
Limitation of liability
To the fullest extent the law allows, Everton is not liable for indirect, incidental, or consequential damages arising from your use of this site. Nothing in these terms limits any liability that cannot be limited by law, including liability for professional negligence in the provision of care.
Governing law
These terms are governed by the laws of the State of California, without regard to its conflict-of-laws rules. Any dispute arising from them will be brought in the state or federal courts located in California.
